EYELID SURGERY

Eyelid surgery, also known as blepharoplasty, is a surgical procedure to improve the appearance of the eyelids. Think about it like eyelid reshaping. The procedure can be performed on the upper eyelids, lower eyelids, or both and can be done in combination with other procedures. In some cases, the muscles that make you frown can be partially removed to permanently smooth the lower central forehead (this is called corrugator muscle resection), which will decrease the need for the use of neurotoxins (Botulinum toxin) in the patients who like the wrinkle correction that these products produce.

Upper eyelid surgery (upper blepharoplasty) removes excess skin and/or fat. In some cases, and contrary to common belief, fat can actually be added in cases where the upper eyes are hollow. In addition, the eyelid may be lifted upwards if needed (see below “upper eyelid lift”)

It is important to notice that standard upper blepharoplasty by itself will not lift the eyelid if a lift is not also performed. There is a misconception, even among surgeons, that removing skin and fat will lift the eye. This is not true, and this is one of the reasons many surgeons do not perform this procedure. Removing skin and fat on the upper eyelid will only reshape the external part of the eyelid and remove hooding but will not lift the eyelid. If the upper eyelid is droopy, an eyelid lift needs to be added to the standard upper blepharoplasty. Blepharoptosis repair consists of lifting the upper eye itself (think about the lower edge of the eyelid being lifted upward to make the eye look larger) by shortening the attachments of the muscle (specifically the tendon) that elevates the eyelid. Also importantly, an eyelid lift can improve your peripheral vision.

Lower eyelid surgery (lower blepharoplasty) is a procedure to improve the appearance of the lower eyelids by eliminating eyelid bags and/or excess skin. Surgery can be done with external or internal eyelid incisions. Excess fat is typically removed or repositioned internally. Occasionally, fat can be added in selected patients if indicated. A common adjuvant to lower eyelid surgery is a temporary or permanent lower eyelid suspension (called canthopexy or canthoplasty) to support the position of the eyelid while it heals. Lasers or peels can also improve eyelid skin texture. A personal consultation will determine what is best for you.
